Pennsylvania drivers now can show their support for the 11,000 local people living with MS by purchasing National MS Society license plate.

To apply for a plate, download this form (PDF, 353 KB) and fill it out. You may be able to type directly into the form before printing. Send it, along with a check for $50 made out to the National MS Society, to:
